Successful Craniotomy done with Excision of Tumor (Enplaque Maningioma)

Abstract
“En plaque meningioma refers to a specific meningioma macroscopic appearance characterised by diffuse and extensive dural involvement, usually with extracranial extension into calvarium, orbit, and soft tissues. These tumours are thought to have a collar-like or sheet-like growth along the dura mater, different from the usual globular meningioma.”* This type of brain tumour is highly vascular and involves risky operation. Being a very rare type of tumour, this needs intensive care. A 45 year old woman with a progressive increasing headache on the right side of the brain for last years, who was also misdiagnosed by a private hospital in Sonepat, visited Park Hospital Panipat. The team at Park Hospital Panipat started the treatment with MRI and brain investigations wherein the En-Plaque Meningioma was diagnosed. There was pain and bulginess in both eye balls due to the tumour and weakness on the left side. The patient was discharged after a week of intensive observation and once we saw recovery signs in her with the ability to move smoothly her limbs.
Background
En-Plaque Meningioma is a rare type of brain tumour and requires intensive care. This is one of the slow growing benign tumours which is attached to the dura and are made up of neoplastic meningothelial cells. These are more common in Western World reporting around 24-30% cases. Meningioma en plaque represents a morphological subgroup within the meningiomas defined by a carpet or sheet-like lesion that infiltrates the dura and sometimes invades the bone. Histopathological features of meningioma enplaque are similar to that of usual meningiomas; however, it is sometimes difficult to predict the behavior in individual cases. Extra-cranial meningiomas form 1-2% percentages of all meningiomas. ** The en plaque variants commonly involve fronto-parietal, juxtaorbital, sphenoid wing, diffuse calvarial or rarely spinal region. *** Due to difficulty in complete resection, the recurrence rate of en plaque meningiomas is higher than the usual counterpart. **** These tumors are also more prone to develop malignant change (11%) when compared to intracranial meningiomas.
Case Presentation
A 45 year old woman visited Park hospital Panipat for an OPD consultation with Dr. Anil Arya wherein he studied the MRI reports and after brain investigations, he diagnosed a rare brain tumour. The lady had progressive increasing headache on the right side of the brain for last 5 years. However, for last 2 years there was pain and bulginess in both eye balls. The patient visited a private hospital in Sonepat for the treatment wherein the CT head was performed and based on the same she was misdiagnosed with Subdural Hematoma. During the operation, the doctor confirmed that the case is of Brain tumour. She was then referred to GB Pant Hospital in New Delhi for further treatment. However, she did not receive any treatment or response; thereafter she contacted Dr. Anil Arya at Park Panipat. After explaining the complexity of the surgery and the poor prognosis during and after the surgery, with due permissions, the patient was operated on 27th July 2017. The tumour mass was completely removed from the brain with craniotomy. Post operation, the patient is conscious oriented with signs of difficulty in limb movement.

Angiography, Angioplasty and Coronary Artery Disease

Coronary Artery Disease is the hardening of arteries that supply oxygenated blood to the heart. These arteries are hardened due to the formation of plaque caused by fatty material and other substances being deposited in the arterial wall. This narrows down the artery and reduces blood flow to the heart and leads to several heart problems, including heart attacks. Heart attacks occur when there is less or no supply of oxygenated blood to the heart muscles causing that part of the muscles to lose function or die.
People suffering from heart problems can be treated using surgical and non-surgical methods. Angioplasty is also one such non-surgical method to treat heart diseases (also called as interventional technique). This method is used to widen blocked or narrowed arteries (a result of coronary heart disease or atherosclerosis) that carry oxygen-rich blood to the heart.

What Happens During Angioplasty Treatment?
Before starting the procedure, some pain medicine and blood thinning medicine are given to the patient. A local anaesthesia may also be given.
Angioplasty procedure: As a part of angioplasty, a cardiac catheterization is performed. A sheath (a thin plastic tube) is inserted through the groin or arm, into an artery. Through this sheath, a long, flexible, narrow and hollow tube called the catheter is passed and guided through the blood vessels to the arteries around the heart. A small amount of a contrast material (dye) is injected into the body to highlight blood flow through the arteries. The dye, injected during angioplasty, is photographed through an X-ray as it travels through the heart chambers, valves and arteries. Through these pictures, the doctor can tell if there are any blockages in the arteries or if they have narrowed down and if the heart valves are functioning properly.
In case a decision is taken to perform an angioplasty, the doctor will move the catheter to the artery with a blockage. An interventional procedure (as mentioned below) will be performed.
Balloon angioplasty: A catheter with a small balloon tip is placed at the point of narrowing of the blocked artery. Upon reaching there, the balloon is inflated to compress the plaque in the artery wall to widen (stretch) the artery and increase blood flow to the heart.
Stent: A stent is a mesh metal tube that is introduced as a scaffold into the coronary artery, with the help of a balloon catheter that is placed over a guide wire. When the balloon tip is inflated, the stent expands to the size of the coronary artery and stays in place permanently.
Rotoblation: An acorn-shaped, diamond coated tip is attached to the catheter, which rotates at high speed and grinds away the plaque upon reaching the blocked artery.
Atherectomy: A minimally invasive surgical method of removing, mainly, atherosclerosis from a large blood vessel within the body. It is generally used to effectively treat peripheral arterial disease of the lower extremities are also been used to treat coronary artery disease
Cutting Balloon: An angioplasty technique that has a special balloon tip with small blade that is activated when the balloon is inflated. The blade chips at the hardened calcific deposits on the artery
What is Angiography?

The patient (an electrician) was admitted to the hospital on 15th June 2017 with a neck bone injury due to his fall off from electric pole. There was severe neck pain and the patient was unable to move his head due to trauma. Although there were no signs of weakness in the limbs and the patient had no signs of breathing issues. Before the major surgery, the patient was given Skull pin traction by the team. Later the Anterior Screw Fixation surgery was done by Dr. Manish Kumar, MBBS, DNB (Neurosurgery) who is a senior neurosurgeon at Park Hospital, Gurgaon.
According to Dr.Manish, this surgery was very complicated as there was neck bone fracture. Also known as cervical spine (neck) trauma. The injury is like decapitation. After investigation and complete examination, nature of the disease and the need for the surgical intervention, along with its limitations and possible complications were thoroughly explained to the patient’s relatives since patient was unconscious. Temporary medications had already been started by that time. After the consent of the relatives, left frontal craniotomy and safe excision of the tumour along with biopsy was carried out. Biopsy proved high grade malignancy of brain.

जांचके उपरांत रोगी के रिश्तेदारों (रोगी बेहोश था) कोबीमारी के बारे में बतलाया गया और ऑपरेशन की जरुरत को समझाया गया. ऑपरेशनऔर उससे होने वाले संभावित खतरे के बारे में विस्तार से समझाया गया. तत्कालअस्थायी इलाज के रूप में दवाएं शुरू की गयी. पूरी तैयारी के बाद 5जून 2017 को स्थायी इलाज़ करते हुए सर्जरी द्वारा ब्रेन कैंसर का इलाज किया गया.  इस सर्जरी में पार्क हॉस्पिटल फरीदाबादमें उपलब्ध अत्याधुनिक तकनीक का प्रमुख योगदान रहा.

डॉ मनीष के मुताबिक इस रोगी में ब्रेन कैंसर बहुत हीं खतरनाक स्थिति में था और बिना सरजरी के जान बचाना मुश्किल था. सरजरी कर पैथोलोजी टेस्ट करवाया गया जहाँ ब्रेन कैंसर होने की पुश्टी हुई. सर्जरी के बाद रोगी धीरे - धीरे स्वास्थ्य हो चुका है. पर डॉ मनीष के अनुसार यह सुधार अस्थायी है और आगे और इलाज की जरुरत है.ब्रेन की यह खतरनाक बीमारी जल्दी हीं रोगी को अक्षम बना देती है.

चिकनगुनिया - डेंगू जैसा एक अधिक खतरनाक रोग

चिकनगुनिया डेंगू जैसी बीमारियों में से एक है, फिर भी यह अधिक हानिकारक है

चिकनगुनिया एक अन्य वायरल बीमारी है जो मच्छर के काटने से फैलती है | डेंगू मच्छर एडेस इजिप्ती और एडस अलबोपिकतस मानव शरीर को काटते हैं और आरएनए वायरस फैलता है (आल्फा वायरस जीनस टूगोवैराइड) जिसके कारण चिकनगुनिया होता है | डेंगू के समान, चिकनगुनिया के लए कोई विशेष इलाज नहीं है, परंतु प्रभावित मानव शरीर को लक्षणों से राहत प्रदान करना | यह रोग पहली बार 1952 में दक्षिणी तंज़ानिया में निदान किया गया था और अफ्रीका, एशिया, भारतीय उप महाद्वीप और लैटिन अमेरिका के क्षेत्रों को प्रभावित करता है |

चिकनगुनिया के कारण:
चिकनगुनिया मुख्य रूप से एडीस इजिप्ती और एडीस अल्बोपिक्टस मच्छरों के काटने के माध्यम से आरएनए विषाणु (आल्फा वायरस जीनस टूगोवैराइड) के प्रसार के कारण होता है |

चिकनगुनिया  के लक्षण:
बीमारी और लक्षणों की शुरुआत आमतौर पर 4 से 8 दिनों तक होती है; हालांकि 2 से 12 दिनों की लंबी अवधि के लिए निरंतर हो सकता है।
  • जोड़ों का दर्द
  • बुखार की शुरुआत अक्सर संयुक्त दर्द के साथ होती है
  • दर्दनाक स्नायु
  • सिरदर्द और पीठ दर्द


चिकनगुनिया बुखार जटिलताएँ:
  •  आरएनए वायरस तीव्र, उप-तीव्र या ख़तरनाक बीमारी पैदा कर सकता है
  • अधिकांश रोगी पूरी तरह से ठीक हो जाते है, हालाँकि कुछ मामलों में संयुक्त दर्द महीनों तक जारी रह सकता है
  • गैस्ट्रॉइंटेस्टैनल शिकायतों के साथ आँख, न्यूरोलॉजिकल और दिल की जटिलताओं के मामले भी हो सकते हैं
  • बुजुर्ग लोगों में, इस रोग से मर्टयू भी हो सकती है

चिकनगुनिया का निदान:
  • सर्जिकल टेस्ट - आईआईजीएम और आईजीजी की मौजूदगी की पुष्टि के लए एलीसा (एंजाइम लिंकड् इम्यूनो-शोषक एसेज़) किया जाता है
  • बीमारी के दौरान आईजीएम के स्तर 3 से 5 साप्ताह के समय में सबसे अधिक हैं और आमतौर पर 2 महीने तक रहता है
  • वायरोलॉजिकल विधि जिसे रिवर्स ट्रांस्क्रिप्टेज़ पोलीमरेज़ चेन रिएक्शन (आरटी-पीसीआर) के रूप में भी जाना जाता है, को भी आरएनए वायरस की उपस्थिति की पुष्टि करने के लिए किया जा सकता है

चिकनगुनिया उपचार:

·         डेंगू के समान चिकनगुनिया के लिए कोई विशेष उपचार नहीं है। रोगी के लक्षणों को दूर करने के लिए उपाय किए जाते है। जोड़ों में दर्द विरोधी प्योरेटिक्स, इष्टतम दर्दनाशक दवाओं और तरल पदार्थ के माध्यम से राहत मिली है। गंभीर क्षति को रोकने में सक्षम होने के लिए रोगी शरीर को हर समय हाइड्रेट किया जाना चाहिए।

डेंगू - मानसून के दौरान एक घातक नाम


पिछले कुछ सालों में, घातक बीमारियों में से एक की वजह से मृत्यु दर बढ़ती हुई हैडेंगू
 
डेंगू, जैसा कि हम सभी जानते हैं, कुछ घातक रोगों में से एक है जो वर्षा के मौसम में उगता है और दुनिया के उष्णकटिबंधीय क्षेत्रों में बहुत से जीवन को दूर ले जाता है। इस रोग से प्रभावित प्रमुख क्षेत्रों में लैटिन अमेरिका, दक्षिण अफ्रीका का हिस्सा और दक्षिण पूर्व एशिया शामिल हैं। हमारे देश के बारे में बात करते हुए, भारत, देश के राष्ट्रीय राजधानी क्षेत्र में 2016 में मृत्यु टोल 1200 तक पहुंच गया, जिससे राष्ट्रव्यापी टोल लगभग 10,000 हो गया। डेंगू वायरस मॉनसून सीजन के दौरान सक्रिय हो जाता है जो जुलाई में शुरू होता है और सितंबर में भारत में समाप्त होता है। इन तीन महीनों के दौरान पूरे देश में डेंगू वायरस प्रभावित होता है। इस लेख के साथ, पार्क अस्पताल ने इस घातक वायरस और बीमारियों के बारे में जागरूकता फैलाने और लोगों को जागरूक करने का प्रयास किया है ताकि लोग लोगों को प्रतिरक्षात्मक उपायों से बचा सकें और डेंगू ज्वर से खुद को बचा सकें।


डेंगू बुखार के कारण: डेंगू का बुखार चार आनुवंशिक रूप से समान वायरस के कारण होता है जो एडीज इजिप्ती और एडीस अल्बोपिक्टस मच्छरों द्वारा मानव शरीर में ले जाते हैं। इन वायरस को मच्छर से मानव शरीर में स्थानांतरित कर दिया जाता है और फिर ऊष्मायन अवधि के दौरान मानव शरीर से मच्छर को स्थानांतरित किया जा सकता है (लक्षणों के सामने आने से पहले)

डेंगू के लक्षण: मच्छर काटने के बाद डेंगू के बुखार के लक्षण लगभग 4 से 15 दिन लगते हैं। आम डेंगू के लक्षण हैं:
बुखार
दर्दनाक स्नायु
हड्डी और जोड़ों में दर्द
सिरदर्द
ठंड / पसीना
चकत्ते / लाल स्पॉट
सूजी हुई लिम्फ नोड्स
पीठ दर्द
भूख की हानि

डेंगू बुखार की जटिलता:आमतौर पर 7 से 10 दिन तक डेंगू बुखार रहता है, हालांकि, कभी-कभी यह अधिक गंभीर बीमारी हो सकती है और विशेष ध्यान और ध्यान की आवश्यकता हो सकती है।
हेमोरेजिक: यह आम तौर पर 15 वर्ष से कम आयु के लोगों में पाया जाता है लेकिन वयस्कों में भी उपस्थित हो सकता है इस स्थिति के लिए प्रमुख लक्षण मसूड़ों और जठरांत्र संबंधी मार्गों में आसानी से चोट लगना है। इस गंभीर स्थिति को डेंगू हेमोर बुखार भी कहा जाता है।

डेंगू बुखार का निदान कैसे करें:मानव शरीर में डेंगू वायरस की उपस्थिति की पुष्टि के लिए सभी अस्पतालों और निदान केंद्रों द्वारा एक सामान्य परीक्षण किया जाता है अर्थात सीबीसी (पूर्ण रक्त परीक्षण)। सीबीसी डेंगू टेस्ट खाते में प्लेटलेट्स और सफ़ेद रक्त कोशिकाओं की संख्या को देखते हैं, जितनी कम होता है, वहीं वायरस का खतरा उतना ही कम होता है। ऐसे हालात हैं, जब सी बी एस टेस्ट के साथ डेंगू की बुखार की पुष्टि संभव नहीं है, क्योंकि लक्षण (बुखार, चकत्ते, सिरदर्द, और निम्न प्लेटलेट्स) कुछ अन्य रोगों जैसे न्यूमोनिया, टाइफाइड और मलेरिया जैसी ही हैं। ऐसी स्थितियों में, वहाँ अन्य परीक्षणों की पुष्टि के लिए उपलब्ध है कि मरीज को डेंगू वायरस से प्रभावित है या नहीं। य़े हैं:
● रक्त और मूत्र संस्कृति
● स्पाइनल टैप
● मैक-एलिसा अट (इम्युनोग्लोब्युलिन एम-आधारित टेस्ट)
● पुलिस महानिरीक्षक-एलिसा
● वायरल पलक कमी टेस्ट
● पीसीआर टेस्ट

डेंगू का उपचार:डेंगू बुखार के इलाज के लिए कोई निश्चित टीकाकरण या दवा नहीं है। इसे आत्म सीमित उपाय से ठीक किया जा सकता है जिसका मतलब है कि देखभाल और आहार परिवर्तन के साथ ठीक किया जा सकता है। रोगी को हर समय हाइड्रेट किया जाना चाहिए और शरीर के दर्द को कम करने के लिए दर्द निवारक दिया जा सकता है। यहां याद रखना केवल एक बात यह है कि गैर-स्टेरायडल एंटी-इन्फ्लोमाटमेंट एजेंट (एनएसएआईडी) को बचा जाना है क्योंकि वे उच्च दबाव ले सकते हैं।
हेमरेहैग्जिक और डेंगू शॉक सिंड्रोम के मामलों में रोगी की स्थिति प्लेटलेट गिनती के साथ बिगड़ती हुई जल्दी से सूखने लगती है ऐसे मामलों में, रोगी को गहन देखभाल उपायों की आवश्यकता होती है जैसे:
● अस्पताल में भर्ती
● आईवी। फ्लुइड हाइड्रेशन
● रक्त परिसंचरण
● प्लेटलेट ट्रांसफ़्यूशन
● रक्तचाप सहायता
